Monday in Bismarck, North Dakota
Having been stuck in Albuquerque last week due to a high winds, I am now in Bismarck where it is awfully cold (10 deg F) and snowy – perfect weather for Sazzad.
We’re here for a couple of days of meeting with the North Dakota state team. We have a whole bunch of really exciting things coming out here (and elsewhere) in January’s 2012.0 release – there should be a beta release of this in a week or so.
Today we’ve been working on Referral, Slot Management, and Overall Service Plans.

Bringing the Circle of Support together in North Dakota

Sazzad and I are here in North Dakota, working with the folks in the State office on the next phase of our implementation.
Thanks to individual unification, this is where it gets really interesting and very exciting (once in a while it ties your brain in knots too!)
We have now developed the Overall Service Plan for North Dakota. This is an annual plan document that can be collaboratively written by state and provider staff, passing back an forward between their accounts in a seemless, HIPAA compliant dance.
We’ll be adding to this an amazing referall process that I think is going to have the same impact on states and counties that T-Logs have had on providers. It’s a very exciting time to be part of Therap.

A very quick trip to Fargo, North Dakota
I left our Side by Side conference in St Louis at the end of the day yesterday to come up here to Fargo for the NDACF Conference. Flying out of St Louis I was able to see quite how much damage was caused to the airport by the recent tornadoes – scary!
The conference here is actually in the same hotel in Fargo where we had our North Dakota Conference last year. Don’t forget that this year we are in Biskmarck on June 21st and 22nd. Sign up here! (schedule coming soon). This is a very impressive conference with over 500 people here from all over North Dakota.
This morning I joined Wendy and Val from the Phase II Steering Committe to talk to folks about what to expect in North Dakota over the coming months. This afternoon I will be presenting on Therap 9.0 and CCHIT.

Justin and Sazzad attending the Buffalo, New York User Group
Sazzad and I have a very busy week coming up. It all kicks off in Buffalo, New York on April 5th when we will be attending a New York User Group Meeting (click on the link to learn more) being hosted by Community Services for the Developmentally Disabiled.
From there we are off to Lincoln, Nebraska to start work on some exciting new projects there and we’ll finish off the trip with some meetings in Bismark, North Dakota.
It’s an exciting time to be part of the Therapverse!

A couple of days in North Dakota

I have spent the last couple of days in chilly Bismarck, North Dakota working with the folks at the Division of Developmental Disabilities on the next phase of their implementation of Therap for States.
A lot of discussion, a lot of thinking, and a lot of progress. Great things are afoot in the great state of North Dakota.
Now all I need is for my plane to leave on time!

Release notes for Therap for States 1.0
We are very excited to announce the release of Therap for States.
Below are the release notes:
Intake-MCI
Intake forms are interfaced with MCI. Users will get the option to check if the user is already in the system and then add the individual into the system accordingly. Users will also get the option to add Involved Individuals or Referral.
Progress Assessment Review (PAR)
- The PAR module assesses individuals using a fixed set of questions.
- The answers to these questions are in multiple choice formats and each answer has a corresponding score.
- The PAR is scored after all the questions are answered and a PAR level is determined for the individual depending on the score.
- Children and adults are assessed under different PARs.
Diagnosis Review
- Diagnosis Review is foundation of Progress Assessment Review (PAR).
- Users can add Diagnosis Reviews for individuals and update them periodically.
- Diagnosis Review is a set of questions that determine if an individual has any chronic or recurrent clinical or personality disorder or any medical condition.
- It also reviews if in case an individual has any of the conditions mentioned above, then does the individual require continued treatment that has a significant impact on the individual’s functioning.
Eligibility
Using this option users can track Individual’s information such as Eligibility, Redetermination of Eligibility and Denial or Withdrawal of Eligibility. This form is used to record information that help determine if a Child or Adult is eligible for waiver. The system also tracks certain areas derived from the PAR that provides the Regional Eligibility Team with information regarding limitations in the seven major life activities during the process of the eligibility determination.

Case Action
Case Action Forms contain data required to define the level of care of an Individual. It allows the use of federal funding for payments of Home and Community Based Services to DD providers. Case Action forms are also created when the level of care changes or ICF/MR provider changes.
Individual Family Service Plan (IFSP)
Therap’s IFSP is used to create Individual Service Plans for child individuals.
The following information are recorded in an IFSP:
- Individual’s present condition
- Family information and concerns
- Risks involved
- Priorities of the service plan
- Expected outcomes from the service plan
IFSP Team
Users will be able to create IFSP Teams for individuals consisting of members who will be selected from the Involved Individual lists associated with the individuals.
Person Centered Service Plan (PCSP)
Therap’s PCSP is used to create Individual Service Plans for adult individuals.
The following information are recorded in a PCSP:
- Individual’s present condition
- Family information and concerns
- Risks involved
- Priorities of the service plan
- Expected outcomes from the service plan
PCSP Team
Users will be able to create PCSP Teams for individuals consisting of members who will be selected from the Involved Individual lists associated with the individuals.
Providers
Users can now create Providers depending on their privileges. Both Organization type and Personal type providers can be added. The module also stores information regarding provider services, provider location and service personnel.
Progress Notes
Information regarding meetings with individuals can be recorded in details using Progress Notes. Notes about significant events during sessions, outcomes and individual’s level of satisfaction regarding the service can also be recorded in this module.
QER Report
This feature will allow users to create reports based on the data collected using Progress Notes. QER Reports can be locked so that the corresponding Progress Notes are no longer editable.
Admin Notes
The Admin Notes can be used to keep a daily log of an individual’s health, activities etc. Users with proper access privileges can create Admin Notes for individuals.
Letter
This feature will allow users to generate letters from specific templates for Individuals and Involved Individuals, The following letter templates will be automatically generated:
- Adult Denial
- Adult Acceptance
- Child Denial
- Child Acceptance
- Redetermination Denial
- Redetermination Acceptance
- Enclosures for Information
- Signature Required Form Enclosed
- Schedule ISP Meeting
- Requesting Signature for ISP
- Notice of Termination for ICF/MR level of care
- Denial of Initial ICF/MR level of care
- PASRR LEVEL II MR/RC Termination of PASRR process

We have included three types of Alerts in the system:
Authorization
There are a set of Authorization forms that are designed to meet the needs of the users in North Dakota. Program Managers can easily Submit the following authorizations for federal funding. The Authorization forms are:
- Self Directed Supports – ASD Waiver Authorization
- Medically Fragile Waiver Authorization
- Self Directed Waiver Authorization
- Traditional Waiver Authorization
- Extended Home Health Care Authorization (EHHCA)

Authorization Template
Central Office Staff members now have the necessary privilege to create Authorization Templates. Once the Authorization Templates have been created, all other staff members except for the Data Administrators will be able to work with the Authorization Templates.
User Types, Roles and Access Control
Therap defines Access Privileges based on different User Types as per the State Requirements. This makes it easier for the Data Administrators to assign Users with different types based on their needs.

- MMIS ISP Report
Data Administration
Data Administrators will have the privilege to create the following:

Therap for States is on its way…
We are very excited to be releasing into production our latest product: Therap for States. Everyone at Therap along with a great team from the Developmental Disabilities Division in North Dakota, led by Director, Tina Bay has put an incredible about of work into this project.
Tomorrow morning we will go live with a system that replaces their existing ASSIST and Lotus Notes application and handles functionality such as intake, identity matching, level of need assessments, case management, infant development, notices, letters, service authorizations and much more.
As tremendous as this is, it’s just the beginning. Phase two of the project will involve integration will involve integration between Therap for States and our regular application which will provide functionality not possible in any other system.
