Guidelines for Sharing Information within Therap
Individuals served on the Developmental Disabilities Waiver (DD Waiver) often have multiple agencies providing services. Similarly, individuals change agencies for all or some of their services. The following document has guidelines on how to share information between two organizations.Please download and read this document carefully.

Register your agency with Therap Services
This is the first step you will need to take. To register with Therap, all agencies are required to complete a Provider Self Provisioning or PSP, which will require entering some basic agency information like address and phone numbers, and identifying who will be your agency’s Provider Administrators (PA). The total process of PSP is explained in a document available here.
With the Self Enroll login, you can login to Therap and create an account for yourself. The Provider Administrator can then login to their account and activate your account. Click here to view the Quick Guide for creating an account using the Self Enroll login. This Quick Guide is specifically for Program Managers or those directed by the Developmental Disabilities Program, to set up a user account in their system.
An Oversight Agency is an organization which oversees other agencies providing services to Individuals. The agencies and the Oversight Agency can share information about the individuals and the services. For example, State DDSD office is an Oversight Agency. State DDSD office oversees providers providing services to Individuals within the state.
The Health Tracking module in Therap allows users to efficiently track different categories of health data and create detailed or monthly reports. It gives a flexible way to record and follow up on medical issues such as Vital Signs, Blood Glucose, and Medication History among others.
With the new Medical Administration Records (MAR) module, users are able to track when a medication has been administered to an individual along with any comments. On the MAR, all current medications for the individual appear on a monthly grid. Users can specify each medication’s scheduled times. Medications can then be administered as per schedule and recorded during each shift. Monthly MAR reports can be generated and exported to PDF and Excel formats. This includes information such as Medication Name, Serving Time, Administered By, and Comments among others. Users can also print a blank copy of the monthly MAR grid or a filled-in grid as needed.
