Comprehensive Health Assessment Tool (eCHAT)
The eCHAT is designed to keep in-depth assessment record of an individual’s health symptoms and history. It also calculates the overall acuity level and publishes a summary with recommendations for cases that may require care plans.
The intuitive eCHAT module automatically pulls in a portion of the information from other Therap modules such as ‘Individual Data Forms’ and ‘Health Tracking’. Like all other Therap modules, the system allows for easy submission, editing, review and reporting.
