Individual Home Page
Therap ‘Individual Home Pages’ allow care providers to find a wide range of information regarding an individual’s background, support plans, health information and activities, all within the same area.
Health Information and Live Feed
‘Individual Home Page’ extracts information from several Therap individual support modules and summarizes them on a page. The first page of an individual home page shows you background data, a picture and a link to the person’s ‘Individual Data Form’ on the left column. Right underneath, is shown a list of their individual service, habilitation and behavior plans and their approval or review dates.
A list of service plans along with recent goal tracking data collection dates are also shown.
The column on the right provides an overview of the individual’s recent health trends by displaying relevant health data, weight, temperature, blood pressure and blood glucose. The checkboxes in the trends section allows you to specify which of these information you want to see on the page.
Individuals’ weight and temperature are displayed on mini graphs. These can be expanded to full size with just a click. This section also links the person’s upcoming appointments & immunization schedules.
The mid section is reserved for live feeds showing new ‘General Event Reports’ (incident reports) and ‘T-Logs’ (day/shift notes) concerning the individual.
Additional Health Information
The ‘Profile’ page of individuals contains additional health information including active medications, diagnoses and allergies. Additional health information, such as height, weight, temperature, pulse, oxygen saturation, blood glucose level and blood pressure are also shown. This page also lists the programs and staff members involved in providing support to the individual.
Photos
The Individual Home Page supports a photo page where you will be able to create photo albums and upload pictures.
