By creating specialized Case Loads, you can support Individuals to document for themselves on things such as GERs, Health Tracking and their medications (MAR). This presentation also uses specialized Case Loads to create cohesiveness between CM’s, Therapists, Guardians, and the Responsible Providers.
The presentation represents how we teach and support Individuals to do their own GERs and medication tracking. Included in this training we would train on how to do “Creating Cohesiveness using Specialized Case Loads” for Guardians, Therapists, and Case Managers. One of the things that we ran across in NM was that the Case Managers had oversight accounts but could not see how we were tracking the Outcomes for Individuals because they were done through the ISP Programs. So, in order for Therap to be as efficient as we were wanting we had to set up Specialized Case Loads to give the Case Managers access (under our account) to track those Outcomes. We did the same thing for Therapists who do not have oversight accounts to be able to follow Individuals’ progress and communicate directly with staff. By creating these specialized Case Loads we have created cohesion between services.
