Therap Services is delighted to invite the users of the state of Ohio to our upcoming webinar titled, Person Centered Planning options in Therap: Charting the LifeCourse and Therap’s ISP Module. The session has been scheduled to be held on November 12, 2020 at 1:00 PM (EST) and will be hosted by Therap’s Assistant Director Business Development Michelle Saunders and Senior Training & Implementation Specialist Gretchen Falk-Lanfear.
Charting the LifeCourse module is a person-centered framework. It is designed to consist of several tools that can provide a comprehensive approach to planning support for individuals with Intellectual and Developmental Disabilities (I/DD) and/or their families.
Charting the LifeCourse currently includes the following tools:
- Life Trajectory
- Integrated Supports Star
- Vision Tool
The ISP Module can be used to train individuals with I/DD and other individuals in the broader HCBS/LTSS settings according to the requirements of the particular desired goal or the valued outcomes to be achieved. Each ISP Program is configurable according to the detailed information such as the long term objective, frequency of documentation, the criteria for completion and the goal to be achieved upon completion of the ISP Program.
