Person Centered Planning options in Therap: Charting the LifeCourse and Therap’s ISP Module in Indiana

Therap Services is pleased to invite the users of the state of Indiana to our upcoming webinar titled, Person Centered Planning options in Therap: Charting the LifeCourse and Therap’s ISP Module. The session is scheduled to be held on November 17, 2020 at 10:00 AM (EST) and Therap’s Assistant Director Business Development Michelle Saunders and Senior Training & Implementation Specialist Gretchen Falk-Lanfear will guide through this event.

Charting the LifeCourse module is a person-centered framework. It is designed to consist of several tools that can provide a comprehensive approach to planning support for individuals with Intellectual and Developmental Disabilities (I/DD) and/or their families.

Charting the LifeCourse currently includes the following tools:

  • Life Trajectory
  • Integrated Supports Star
  • Vision Tool

The ISP Module can be used to train individuals with I/DD and other individuals in the broader HCBS/LTSS settings according to the requirements of the particular desired goal or the valued outcomes to be achieved. Each ISP Program is configurable according to the detailed information such as the long term objective, frequency of documentation, the criteria for completion and the goal to be achieved upon completion of the ISP Program.

Tentative speakers:

Jeff Case is the National Director of Business Development at Therap. He has worked with non-profits for the past 30 years. He works with provider agencies and State organizations throughout the Central United States and lives in the Kansas City, MO area.

Leah Sewell is a Senior State Implementation Specialist at Therap. She has worked in the Human Services field since 1999, providing comprehensive services to individuals with developmental disabilities and mental health diagnosis. Prior to joining Therap, Leah used Therap system for documentation of supports and team-wide communication in residential settings in Southern Maine.

Jeff Covington is the Deputy Director of State Implementation at Therap. He has been working in the I/DD industry since 1995. His role is to oversee the implementation of Therap for States and he comes with extensive experience in project management and leading a diverse team to successful outcomes.

She has worked in the non-profit sector with agencies supporting persons with Intellectual and Developmental Disabilities and Mental Health diagnosis since 1995. Michelle was instrumental in implementing Therap at her agency and also completed grant applications and led statewide training efforts for the Missouri College of Direct Support.

Catherine Hauser is a Training and Implementation Specialist at Therap. Her experience in the developmental disabilities field began in 2005 in Illinois as a Direct Support Person. Catherine introduced Therap to her agency and was able to see both the immediate and long-lasting benefits offered by all of Therap’s services.

Caiti Woodburn is a Training and Implementation Specialist at Therap. She has been working in the developmental disabilities field since 2008. Prior to joining Therap, Caiti has worked as a Direct Care Staff and also had experience as a user and provider administrator of Therap system

About the presenters:

Stacy Wilson is a Business Development Consultant at Therap. She began her work serving individuals in the Intellectual and Developmental Disabilities (I/DD) field in 2007 as a Direct Support Professional, followed by roles in program coordination, and most recently serving as the Assistant Director at Harmony House Care Homes in Blue Springs, Missouri. She led the transition to a paperless system at HHCH and has first-hand experience with how much of an impact Therap Services can have at every level of service delivery within an organization.

Bonnie Sorrells is a Training and Implementation Specialist at Therap. She previously worked for Ouachita Enrichment Centers in South Arkansas as The Director of Adult Services and HIPAA Coordinator for 15 years during which she received training in HIPAA, behavioral planning, case management, medication management, and supported employment. Bonnie has been working with the Therap system at her prior agency since 2013 as the administrator and primary Therap in-house trainer.
