We are very excited to announce the release of Therap for States.  

Below are the release notes:




Intake forms are interfaced with MCI. Users will get the option to check if the user is already in the system and then add the individual into the system accordingly. Users will also get the option to add Involved Individuals or Referral.

Progress Assessment Review (PAR)



  • The PAR module assesses individuals using a fixed set of questions.
  • The answers to these questions are in multiple choice formats and each answer has a corresponding score.
  • The PAR is scored after all the questions are answered and a PAR level is determined for the individual depending on the score.
  • Children and adults are assessed under different PARs.

Diagnosis Review



  • Diagnosis Review is foundation of Progress Assessment Review (PAR).
  • Users can add Diagnosis Reviews for individuals and update them periodically.
  • Diagnosis Review is a set of questions that determine if an individual has any chronic or recurrent clinical or personality disorder or any medical condition.
  • It also reviews if in case an individual has any of the conditions mentioned above, then does the individual require continued treatment that has a significant impact on the individual’s functioning.




Using this option users can track Individual’s information such as Eligibility, Redetermination of Eligibility and Denial or Withdrawal of Eligibility. This form is used to record information that help determine if a Child or Adult is eligible for waiver. The system also tracks certain areas derived from the PAR that provides the Regional Eligibility Team with information regarding limitations in the seven major life activities during the process of the eligibility determination.

Case Action



Case Action Forms contain data required to define the level of care of an Individual. It allows the use of federal funding for payments of Home and Community Based Services to DD providers. Case Action forms are also created when the level of care changes or ICF/MR provider changes.

Individual Family Service Plan (IFSP)



Therap’s IFSP is used to create Individual Service Plans for child individuals.
The following information are recorded in an IFSP:

  • Individual’s present condition
  • Family information and concerns
  • Risks involved
  • Priorities of the service plan
  • Expected outcomes from the service plan




Users will be able to create IFSP Teams for individuals consisting of members who will be selected from the Involved Individual lists associated with the individuals.

Person Centered Service Plan (PCSP)



Therap’s PCSP is used to create Individual Service Plans for adult individuals.
The following information are recorded in a PCSP:

  • Individual’s present condition
  • Family information and concerns
  • Risks involved
  • Priorities of the service plan
  • Expected outcomes from the service plan




Users will be able to create PCSP Teams for individuals consisting of members who will be selected from the Involved Individual lists associated with the individuals.




Users can now create Providers depending on their privileges. Both Organization type and Personal type providers can be added. The module also stores information regarding provider services, provider location and service personnel.

Progress Notes



Information regarding meetings with individuals can be recorded in details using Progress Notes. Notes about significant events during sessions, outcomes and individual’s level of satisfaction regarding the service can also be recorded in this module.

QER Report



This feature will allow users to create reports based on the data collected using Progress Notes. QER Reports can be locked so that the corresponding Progress Notes are no longer editable.

Admin Notes



The Admin Notes can be used to keep a daily log of an individual’s health, activities etc. Users with proper access privileges can create Admin Notes for individuals.




This feature will allow users to generate letters from specific templates for Individuals and Involved Individuals, The following letter templates will be automatically generated:

  • Redetermination Denial

Authorization Template



Central Office Staff members now have the necessary privilege to create Authorization Templates. Once the Authorization Templates have been created, all other staff members except for the Data Administrators will be able to work with the Authorization Templates.

User Types, Roles and Access Control



Therap defines Access Privileges based on different User Types as per the State Requirements. This makes it easier for the Data Administrators to assign Users with different types based on their needs.
