Health Care Reports

One of the major strengths of Therap’s health information tracking system is its powerful reporting features. A good example of this is Therap Health Care Reports which aggregate and summarize health information from different sections of the Health Tracking application (e.g. vital signs, height/weight, respiratory problems, medication history, lab test reports etc.), as well as other modules such as General Event Reports and Individual Data. Here are some of the useful features supported by the report –

Flexible parameters
The report supports flexible options to select date range and select report sections. Report sections may include some or all of the health tracking records.

Clean summarized view

All information is arranged neatly in simple, tables.

Printable PDF format
The reports can be viewed and saved in printer friendly PDF format.

Saving for later use
Once drawn up, users are able to save them within the system to use later.

Comments & recommendations
Relevant comments and recommendations can be saved with the health care reports.

Medication History Custom Reports

The Medication History module of Therap’s electronic Health Tracking application supports a standard reporting feature that allows users to produce reports on an individual’s approved and/or discontinued medications within a specific period. The report includes such details as medication category, subcategory, type, dosage, measurement unit, frequency, route, prescriber’s name and many more.

In addition to this standard report, Therap Medication Histories allow users the flexibility to create custom reports in which they are able to specify the output columns, for medication names, category, dosage, frequency, administration frequency and so on. The custom reports can be based on parameters such as particular individuals, programs, medication administration date and medication type among others.


As with other Therap reports, the Medication History custom reports can be exported to Excel for further data analysis and manipulations.

Adding multiple medications to Medications History

The Medication History module of Therap Health Tracking maintains relevant information regarding an individual’s current and previous medications such as dosage, form, count, and frequency.

The module offers the flexible option for users to add multiple medications to a person’s Medication History by importing them from an Excel spreadsheet.

The procedure is simple – just select the Import from Excel link under the Medication History from the Health Tracking module. This will open the Medication History Excel Import page. Here you will find a link to a sample Excel file that you can use to upload your medications.


Now, download the Excel file and enter your medications on it. (Do not change the header cells on the spreadsheet.) Once you are done, just go ahead and attach it on the ‘File’ field.

All you need to do then, if to fill out the form and hit ‘import’.

Creating Medication History for an Individual

The Medication History section in Therap’s Health Tracking module is used to keep records of medication(s) given to Individuals in developmental disabilities.

An individual’s medication history can be recorded the following way:

  1. Click the ‘New’ link under the Medication History section on the Health Tracking area on your FirstPage.
  2. Select the appropriate program for entering medication from the list of programs available.
  3. The Medication History form will open. On ‘Section 1 – General Information’ section of this page, select the individual for whom you want to add medication from the ‘Individual Name’ dropdown list. When you do this, a link will appear which will allow you to a list of medications prescribed for this person.
  4. On ‘Section 2 – Use the drug look up feature which lets you get detailed information about a drug from the First DataBank database. You can add medications directly from this list.
  5. You may also enter other information, such as dosage, measurements, frequency and side effects.
  6. Enter the date when the medication will start.
  7. Once you have finished adding information click the ‘Submit’ button at the bottom of the page to submit the form. You may use the ‘Save’ button if the form has not been fully completed and needs to be saved for later use. Please note that you must enter all the information in the fields that are marked with an asterisk (*).
