New in 2012.0 – Medication Reconciliation

In version 2012.0, Therap is bringing out a new module to track individual Medication Reconciliation.  The module will track whether that medication reconciliation has been completed at the time of admission, discharge and each change in level of care during facility stay. The module will provide the ability to capture a signature or the User ID and the date stamp as per the CCHIT requirements.

While adding reconciliation on the form, the application offers flexible options for users to select the reconciler’s name from the user list, as well as physicians and Registered Nurses’ list pulled in from contacts. Users then include the reconciliation date, specify active duration, attach additional documents and relevant comments.

The introduction of the Medication Reconciliation module (along with Medication Profiles, Individual History and Care Plans) is consistent with Therap’s goal to achieve CCHIT compliance by mid 2012. For more information on Therap’s CCHIT Compliance Plan, please refer to our CCHIT Requirements page.

CCHIT and Meaningful Use of EHR

CCHIT certification has gained much importance of late in setting the standards for electronic health records (EHR). No doubt you’ve been hearing about it. Put it simply, it is a standard set by an independent non-profit which has been approved by Approved by the Office of the National Coordinator for Health Information Technology (ONC) of the U.S. Department of Health and Human Services (HHS).

With a goal to be CCHIT compliant Therap electronic health records now support medical diagnoses, advanced directives, vital sighs (with normal range definitions) and allergy profiles. Also work is in progress for Consent And Authorization Directive, Patient History, Medical Reconciliation, Administrative/External Clinical Documents (privacy policy, release information, faxes, referral authorizations, consultant report etc.). The plan is to be fully CCHIT compliant by 2012.

If you have questions regarding CCHIT compliance and meaningful use of EHR, a good place to have them answered would be Therap’s regional conference in Kissimmee, Florida on December 6-7, 2011. In addition to discussing CCHIT and EHR, the event will have regular discussion and training session for existing and prospective users of the Therap system. You can learn more about this event at https://www.therapservices.net/conferences/.
