Intro to the New IDF

Therap Services welcomes you to the upcoming webinar titled “Intro to the New IDF” that is scheduled on June 04, 2020 at 11.30 AM (EST).

The purpose of Individual Data form (IDF) is to help you record and maintain essential Information regarding the individuals you support. The forms consist of basic information such as names, gender, date of birth, SSN, Medicaid number, admission date, physical features, and more. Letisha Ulmer is currently Assistant Director of Quality Assurance who joined the Therap Training and Implementation team in 2014. She has worked in the Human Services field since 2008 in the state of New York where she filled professional roles including a Therap administrator, a residents’ benefits clerk and a day habilitation secretary.

Catherine Hauser is a Large Provider Project Manager who joined Therap Services team back in 2017. Her experience in the developmental disabilities field began in 2005 in Illinois when she became a Direct Support Person. Shortly after, Catherine became a QIDP for both ICF/DD and CILA programs at her previous agency.

Elizabeth Stanton is a Training and Implementation Specialist of the he Therap Services team who joined in 2020. Liz has previous experience working with a large provider in New York State for 16 years; holding various roles, with the most recent being the Therap Administrator for 8 years.

Therap Services is pleased to announce the upcoming webinar on “Using the Individual Support Plan for Comprehensive Annual Planning” that is scheduled on May 13, 2020 at 01:00 PM (EST).

Therap’s Individual Support Plan module is designed for you to plan and document individual supports. Introduction to Time Tracking

The Time Tracking module is available for everyone as it is a part of the ‘Therap Basic Package’. You and your agency staff can all enjoy the benefits from using this module to document data timely on several significant areas for your individuals that require monitoring. You may want to track your individuals’ moods, sleep, behaviors, toileting, repositioning, bed checks, community inclusion/outings, accountability, restrictive device, intake/elimination, adaptive equipment and more! You can create Time Tracking Templates as needed and record Time Tracking Data to generate reports, all of which can assist you to ensure greater developments in the well-beings of your individuals.

Bonnie Sorrells has been the Training & Implementation Specialist at Therap since 2018. Prior to this, she had been fulfilling her roles as The Director of Adult Services and HIPAA Coordinator at Ouachita Enrichment Centers in South Arkansas. Bonnie has been working with the Therap suite of applications since 2013 as the administrator and primary Therap in-house trainer at her previous agency.

Milagros “Mimi” Roche is presently the Training & Implementation Specialist at Therap. Her journey with Therap began in 2016 when she first joined as a Bilingual Training Specialist. Mimi has over a decade of experience working passionately in the field of Development and Intellectual Disabilities. She has adopted the use of the Therap System since 2005 in the many roles she has been fulfilling since that time.
