The purpose of Individual Data form (IDF) is to help you record and maintain essential Information regarding the individuals you support. The forms consist of basic information such as names, gender, date of birth, SSN, Medicaid number, admission date, physical features, and more. Know the Presenters

Letisha Ulmer is currently Assistant Director of Quality Assurance who joined the Therap Training and Implementation team in 2014. She has worked in the Human Services field since 2008 in the state of New York where she filled professional roles including a Therap administrator, a residents’ benefits clerk and a day habilitation secretary.

Catherine Hauser is a Large Provider Project Manager who joined Therap Services team back in 2017. Her experience in the developmental disabilities field began in 2005 in Illinois when she became a Direct Support Person. Shortly after, Catherine became a QIDP for both ICF/DD and CILA programs at her previous agency.

Overview of Individual Data Form

We are pleased to announce our upcoming webinar on ‘Overview of Individual Data Formwhich will be held on February 13, 2020 at 12:00 PM ET. The webinar will provide an overview on the Individual Data Form module.

The Individual Data form (IDF) is designed to help users maintain essential information such as race, religion, SSN, Medicare, Medicaid number, admission date, program enrollment date, etc. for individuals, along with their basic identification details such as name, date of birth, physical features, and others. Users are able to attach to this module two photographs and scanned documents and store them in a HIPAA compliant form.

About the Presenters:

Bonnie Sorrells joined the Therap Services team as a Training and Implementation Specialist in 2018. She previously worked for Ouachita Enrichment Centers in South Arkansas as the Director of Adult Services and HIPAA Coordinator for 15 years during which she received training in HIPAA, behavioral planning, case management, medication management, and supported employment. Bonnie has been working with the Therap system at her prior agency since 2013 as the administrator and primary Therap in-house trainer.

Katy Lilly joined Therap in 2018 with five years of experience in the I/DD field. She began as a Direct Support Professional at United Cerebral Palsy of Greater Birmingham supporting individuals with physical, developmental, and intellectual disabilities in a Day Program setting. Katy assisted with the initial setup of Therap, staff training, and quality assurance of documentation for the company.

Demographics in Therap: IDF/EDF and the Demographic Dashboard

We’re pleased to announce our upcoming free webinar on ‘Demographics in Therap: IDF/EDF and the Demographic Dashboard‘ which will be held on 14th May 2018, at 1 pm EST/10 am PST. Participants in this webinar will get an overview of Therap’sDemographic Dashboard features. Moreover, they will also learn how they can use the module to record information for the individuals that they support.

The webinar will be conducted by Bonny Sorrels and Rich Frettoloso.

Therap Training Academy Course on Individual Data Form Overview

Therap announces new Training Academy course on Individual Data Form Overview. This course will give you an overview of Therap’s Individual Data Form (IDF) and Emergency Data Form (EDF) sections. Every supported individual at an agency would have these two forms connected to them. This will cover what they are and how you can enter data into them.

This course is designed for users who are responsible for entering and/or viewing data on the Individual Data Forms.
