The admission or individual intake process in Therap starts when the Provider Administrator at an agency enters a new individual’s basic demographic information when there’s an oversight agency, a user also assigns a unique Oversight ID. The user is then taken to the Individual Data Form where detailed information related to providing care to the individual is entered.

Therap’s Individual eligibility determination process helps providers to record information to determine if a person is eligible for waiver services. The Individual Eligibility form can be used to complete redeterminations.

Therap 9.1: Individual Intake and Referral

With Therap 9.1, released yesterday, an oversight agency, such as a State department of developmental disability services, or a multi-agency support provider will have an effective system to handle individual intake and referrals through Therap.

Users at the oversight agencies will be able to intake individuals into the system by filling out a brief ‘Individual Entry’ form. While doing this, users will have to assign an ‘Oversight ID’, a unique identification that will be used to link individual records across multiple linked providers.

Once this form is saved, an ‘Individual Data’ form will be created for the individual taken into the system.
Again, individual intake can also be done by support providers linked to oversight agencies. They would do this by filling out a similar ‘Individual Entry’ form. They will not assign the ‘Oversight ID’ for the individual. They will have the option to select one or more (if present) oversight agency for the individual.

Once the oversight agency intakes new individuals into the system, it can send referrals for them to one or more linked providers. Oversight agency users will be able to access this option from the ‘Multi-Provider Individual’ section on their FirstPage.

Linked Providers will find these referred individuals on the Pending Admission list on their FirstPage.
