The discussion will chronicle our paperless journey from poorly written documentation to quality documentation that is HIPPAA compliant and easily retrievable. Lastly, this presentation will highlight the results of audits pre-Therap and post-Therap.

I will have a presentation with examples of how we collected and prepared our old documentation (on paper) parallel to documenting within Therap. For example, I will highlight daily documentation issues prior to Therap such as, paper documentation lacking date and time stamps and verification of completion; the struggle of communicating between departments, and the learning curve of my staff. Additionally, I will highlight the importance of utilizing your Therap trainer during each phase of implementation. I want to let everyone know how helpful our Therap Trainer has been; how no one has just left us hanging. Overall, we have been “live” since November 1, 2014; and I want to note that in less than one year, we as an agency are leaps and bounds beyond our initial paper “struggle”.

Ryan Colin from Life Works will be presenting at the 2016 Therap National Conference.

Ryan will discuss the agency experience at Life Works and what it took for staff to come on board. At first our staff were very resistant but overtime we have worked hard to make our books paperless and we’ve made wonderful progress. Even with current projects staff can give some push back but with communication and cooperation we’ve been able to move forward successfully.

I will demonstrate how we complete the CHAT and what information is put in the CHAT. I will also discuss the use the Health Passport for hospital visits to ensure the information is available to provide the best care.

Overview of the CHAT and Health Passport and how it can be used in the nursing aspect of the care of people with developmental disabilities will be the main focus.

Using Therap to Track Treatment Fidelity and Improve Client Outcomes: User Presentation, 2016 Therap National Conference

Omni BManagement staff will utilize Therap to track staff compliance in three areas (t-log completion, ISP data, and BSP program fidelity) to determine the impact of increased compliance has upon client behavior. Additionally, over three months management will introduce multiple levels of feedback for staff to determine if that increases compliance in the above identified areas which may potentially impact client behavior. It is believed that as staff compliance improves (through feedback and group contingencies/incentives) that client maladaptive behaviors will inversely improve.

This information will be tracked in two different programs (EFH & group home) to demonstrate how it can be used in a variety of settings to encourage overall agency compliance and improvement.

This session will give a detailed account of the process of tracking fidelity and improving client outcomes though therap tracking, one on one staff feedback, and group feedback.

Using Therap to Keep Track of Client Medical Appointments and Health History, Including MARS: User Presentation, 2016 Therap National Conference

You will learn how to keep track of appointments and doctor’s orders, including medication changes. You will learn how to link medication changes and history to your MARS, IDF and EDF. You will learn how to run Health Care Reports that you can print off and take to doctor’s appointments such as seizure records for the neurology appointments, food/fluid intake to primary care or nutrition appointments, blood glucose to take to endocrinologists, etc. All of these will help to improve the care and oversight that your clients receive.
