About the presenters:

Joshua Corzantes is a Senior Training & Implementation Specialist at Therap. Prior to that, he worked for 11 years in the field of persons with intellectual and developmental disabilities.  He had also used Therap’s software for a year as a Provider Administrator and gained a vast knowledge of the application.

Catherine Hauser is a Training and Implementation Specialist at Therap. Her experience in the developmental disabilities field began in 2005 in Illinois as a Direct Support Person. Catherine introduced Therap to her agency and was able to see both the immediate and long-lasting benefits offered by all of Therap’s services.

Therap Mobile Application for Disability Providers Allows for Point-of-Service Documentation

Therap mobile app for Android and iOS enables support professionals working in the home or community to navigate service programs for the individual they support and remotely collect outcomes and service data using the ISP Data app. With the T-Log app, staff can directly submit shift notes or progress notes from their phone at the point-of-service. The Medication Administration Record app permits staff passing medications to record administration, notes, and status immediately following the med pass. They may also view schedules, along with active diagnoses, allergy records and the contact information of the prescribing physician. Therap’s Scheduling/Electronic Visit Verification (EVV) module has been designed to meet the needs of states and providers who need to build staff schedules (especially for in-home and community-based services), track staff hours, monitor individual service allocation and meet the upcoming federal EVV requirements. Additionally, supervisors working remotely may also reset passwords using an app to facilitate seamless access in documenting on Therap.

The tools available within the selection of Therap mobile apps allow Program Managers and Quality Assurance Directors to view data recorded at point-of-service and evaluate the quality of those services remotely.  App users may take photos of the service, both date and time stamped. For HIPAA security, photos do not save to the individual device but are directly uploaded to Therap. Each action taken by the user submitting data is coupled with an electronic signature.  Along with GPS location tracking, this provides multi-level data authentication and allows for monitoring quality of services provided to increase accountability, transparency and reduce or eliminate give-back.

Therap Electronic Visit Verification

In compliance with Federal and State Electronic Visit Verification requirements, Therap’s EVV technology is helping facilitate service delivery authentication processes for Therap users. EVV data collected via web and mobile applications are integrated with the various Therap modules, forming a comprehensive Electronic Health Record for individuals being served through Therap.

In these instances providers can utilize an integrated documentation solution with EVV capabilities. The Therap’s system is based around the theory that data should be collected and entered into the system by the person who directly provided the service as close to the point of service as is possible in terms of time and location. Therap’s mobile app comes with tools that allow for GPS location tracking, electronic signatures and date and time stamps in addition to photo uploading options for recording particular activities. These features work together to help administrators authenticate service delivery through the various notification, reporting and tracking features available on Therap. As the Therap mobile app is part of a larger system that forms a comprehensive EHR, EVV data collected via the mobile app transfers over seamlessly and integrates with various modules on Therap.
Therap’s implementation of Electronic Visit Verification helps authenticate service delivery through:

  • Tracking type of service provided
  • GPS location tracking
  • Electronic signatures of staff and service recipients
  • Date and time stamps of service provided
  • HIPAA compliant photo uploading

The same EVV data is fed directly into the billing system, resulting in robust audit protection. Additionally, Therap implements EVV technology across the system to collect detailed information about the specific types of services being provided. The system uses a standard web browser as the foundation for data collection. Users access Therap by logging into the secure system from their devices and authenticating themselves using their unique login credentials. From this point on, the system, as part of the documentation process, collects and record necessary information based the specific actions that the user performs in the system. The system also has the ability to collect the user’s IP address, thus providing information regarding the user’s location during the time that staff is out in the home or community setting.

Beyond Version 5010: ICD-10 Compliance


As of January 1, 2012, health care providers, payers, software vendor (e.g., Therap), and clearinghouses/third-party billers – must conduct all electronic HIPAA transactions using Version 5010 standards, according to CMS requirements.

Therap’s electronic billing system is ready to handle 5010 transactions. Therap is, and will continue to provide necessary support and training to all our users.

Looking onward the next task is to move on to ICD-10 codes for medical diagnosis and inpatient procedures by October 2013. Between then and now, a number of milestones will have to be achieved by everyone involved. A detailed timelines of activities on the path to Version 5010 and ICD-10 compliance, is provided by CMS here. CMS is also offering a desktop and mobile app for this purpose that you should definitely check out.
