The Electronic Comprehensive Health Assessment Tool or eCHAT is an in-depth assessment health evaluation of an Individual. It is a simple, easy to understand online assessment of an Individual’s health symptoms and history. It calculates overall acuity level and publishes a summary with recommendations for care plans.

The intuitive eCHAT module automatically pulls in a portion of the information from other Therap modules such as Individual Data Forms and Health Tracking. The features of this automated module include:

  • Improved identification and documentation of health needs
  • Ability to record and view all the health related information in a single form
  • Checklists on an Individual’s major safety issues and risk factors
  • Summary Report that evaluates an Individual’s Health Assessment record
