Have you noticed the handy feature added to Therap’s list of helpful analysis reports? The Service Verification Form is found at the bottom of a created ISP Data Report (via the “Export to” link). At a glance you can see who worked with an individual, when and where on a particular goal. It is also helpful in detecting trends or mistakes in time recording and goal implementation strategies.

I did have a few questions about it as well and thought we might all learn more together.
When printing off do you only have the access to send to adobe? It would also be nice to have a signature line for everyone listed that provided service in the time frame so they can all sign the same document. I also wasn’t sure what the 2nd signature line with “agency: _____” was for as of course they would be from our agency.
Anyway, thanks for the update on the changes. Looking forward to trying more things and figure out the best way to use this.

Good questions. The Adobe format is used to accommodate the individual-specific header information that is automagically included at the top of each printed page. (E.g. Full name, Medicaid number, DOB, etc) The Adobe form can be transmirred via S Comm or attached to antoher Therap form.
We use this form for analysis and not as an official form. As such we dont’ require handwritten signatures. (The original documentation bears the electrnoic signature of the service provider who attests that the information is correct.) However, if we were to use the signature provision at the bottom of the Service Verification Form we would use the top signature for person served, or other nonagency representative and the second signature for agency personnel.
